Job description
  • Provide initial, ongoing, comprehensive assessments of the patient’s needs including all OASIS time points
  • Assist in development and implementation of the interdisciplinary patient specific plan of care
  • Provide interventions and goals to improve therapeutic speech and swallowing difficulties, cognitive re-training.
  • Attend interdisciplinary meetings, staff meetings, in-services per agency requirements
  • Document timely for all visits
  • Communicate weekly with care team

Administrative Assistant (Weekends)

Position responsibilities
  • Greet people as they enter the Agency, answer calls, answers general information questions, screens visitors and delivers messages.
  • Type memos, correspondence, agendas and reports as required.
  • Processes physicians orders and follows up on physician orders.
  • Enters intakes and scans patient documents into EMR.
  • Maintain filing and scanning for the Agency.
  • Open and sort mail.
  • Photocopying as required.
  • Perform other duties as required.
  • Position requires good organizational skills and time management.
  • It is primarily a desk job, which essentially involves sitting, standing, stooping and walking, as well as an inordinate amount of telephone communication.

Start of Care Nurse (SOC) – Full-time

We are looking to fill the position of Monday – Friday Start of Care Registered Nurse.
